Born at Lubaga Hospital, extending care beyond walls

HomeBridge+ was founded in 2024 as an autonomous but Lubaga Hospital-born project, designed to extend the hospital's century-old philosophy of "care for life" beyond the facility walls into homes and communities.

Lubaga Hospital has, for over a century, been at the heart of healthcare delivery in Uganda, serving as one of the country's leading faith-based referral hospitals. Yet, despite these efforts, gaps remained in the continuity of care—especially after patients were discharged, among postnatal mothers, in preventive wellness, and among patients living with chronic non-communicable diseases.

HomeBridge+ emerged to bridge these gaps, functioning independently while staying deeply aligned with Lubaga Hospital's mission and vision—bridging the gap between hospital-based care and home-based recovery.

The NCD Crisis

33% of all deaths in Uganda are caused by NCDs. Every Ugandan has a 22% probability of dying prematurely from cardiovascular disease, diabetes, cancer, or chronic respiratory disease.

Maternal Health Gap

Only 33.3% of mothers receive a postnatal check within the WHO-recommended 48-72 hours after childbirth, leaving most mothers and newborns without critical follow-up care.

Neonatal Mortality

Neonatal mortality stands at 22 deaths per 1,000 live births—nearly double the SDG target of 12. Most deaths occur within the first week after birth.

Readmission Crisis

12% of adult medical patients are readmitted within 30 days. Heart failure patients face 17-27% readmission rates within a month of discharge.

Critical Delays

28% of perinatal deaths are linked to delays in seeking care, and 23% to delays in reaching health facilities—gaps HomeBridge+ bridges through home visits.

Institutional Challenges
